Each TRICARE Prime member is assigned to one of seven clinics for their Primary Care: Evans Family Medicine Clinic, Pediatric Clinic, DiRaimondo Family Medicine Clinic, Robinson Family Medicine Clinic, Warrior Family Medicine Clinic or the Premier Health Clinic. Representatives at the clinics or appointment line can assist with booking a school physical.

The top one (health assessment form) is the one for school physicals. It is good for one year.
